Prescription Refills

In our ongoing effort to make your pet's health care as convenient and easy as possible, you can now request a refill for your pet's prescription by submitting the following form. Please be sure to fill in all the requested information. The prescription refill must be approved by a doctor.

In order to fill the prescription, one of our doctors must have examined your pet within the past year & some medications require blood tests in order for the prescription to be filled. Our testing requirements are as follows:

Heartworm prevention: heartworm test once a year
Arthritis medication: liver profile every 6 months
Thyroid medictions: thyroid level (T4) every 6 months
Seizure mediction: phenobarbital or bromide level every 6 months
